To set up your information in our database, we require:

bulletThat you fill out the carrier information form.
bulletA copy of your W-9.
bulletA copy of your authority.
bulletA copy of your cargo insurance.
bulletSigned broker agreements.
bulletThree alternate land line telephone numbers(if you run fewer than 11 trucks) ***
bulletSigned fuel advance authorization(if a fuel advance is requested)

*** Note: alternative numbers are requested to communicate with dispatch or a driver in transit if a carrier has 10 trucks or less. It is in our experience, that often, these carriers do not have a full time stationary dispatcher at a location from 8 am-5 pm.  Cell numbers are subject to change, frequently due to rates, service and reception.  Our customers expect a rapid and knowledgeable response to the freight in transit and delivery of their products.   Therefore, we require 3 alternate land line telephone numbers.  These numbers could be residences, partners, owners, or relatives.  Alternate contacts will not be called unless it is absolutely necessary.
